Late Cancellation / No Show Policy
Please cancel or reschedule your appointment 24 hours in advance if you’re not able to attend.
A Late Cancellation or No Show $80 fee is charged the same day when 24 hours notice is not given when cancelling or rescheduling an appointment.

Intake Appointment
The first session is a time to meet each other, discuss your concerns and determine whether we're a good fit. For Children, the first appointment is typically Only with the parents. For Teens, some of the session time will be reserved just for the Teen, although both the teen and parent(s) need to attend.
Two Easy steps to Treatment
Message us & provide Insurance iNfo
Be prepared to provide your insurance information (Carrier, Plan name, Policy ID, Group number, and the insurance Subscriber’s name and date of birth) and a brief reason for treatment. Contact our office with any questions.
2. Complete the Intake Forms
When there is availability, we will contact you from the waitlist to schedule an appointment. New clients will receive a “Welcome Email” with your private link to the secure Client Portal. All new clients must sign the required documents, fill out necessary information, place an active credit card on file, and upload photos of the front and back of your insurance card. You can obtain a new direct link by following this Client Portal button below once your account has been activate. Please be sure to complete these steps in entirety within 48 hours to keep the tentatively scheduled appointment. After verification of your insurance benefits, you will receive a letter with the Estimated Insurance Benefits Coverage Quote given by your insurance plan. Appointments will be rescheduled if the intake forms, credit card information and insurance card uploads are not completed within 48 hours.
